Becoming an Electronic
Claims Provider

The following are forms you will need to become an Electronic Claims Provider:

  • Electronic Media Addendum (MAP 380)

  • 835 Request Form

  • BBS EDI Application

  • Dialup User Manual

    Review Software Vendor and Billing Agency lists. (divided by billing format) Available by contacting the EDI Helpdesk at : 1-800-205-4696.

    Purchase computer equipment (if neccessary) and choose the method of submission that best fits your needs.

    Know your options.

    If you are a large volume Medicaid provider with large volumes of other insurance to bill, you may choose to speak to your current software vendor to determine if they have a module available to you which will integrate with your current accounting software. We can supply you current electronic claims files specifications to forward to your vendor, or corporate office.

    If you are a small to medium Medicaid provider, you may wish to choose a software vendor or billing agency, who may supply you with modules that will integrate with your current accounting software, or you may choose to request our free software which is available to all providers.

    If you have your own programmers available, you may wish to request a copy of the Companion guides to assist in writing your own program.
